Transitioning from Pediatric to Adult Care with Anna Gravelle and Kristine Kerr
On July 22nd 2015, Cystic Fibrosis Canada hosted its fourth educational webinar in our Virtual Education Program for Patients and Caregivers. Anna Gravelle and Kristine Kerr each discussed, ‘Transitioning from Pediatric to Adult Care’.
Anna has been the Cystic Fibrosis Nurse Clinician/Clinic Coordinator at British Columbia’s Children’s Hospital (BCCH) since 1993 and is adjunct faculty at the University of British Columbia School of Nursing. From 2000 to 2008, she led a CF quality improvement initiative at BCCH. This initiative resulted in a multidimensional CF transition program, including a CF transition care clinical pathway and Readiness to Graduate questionnaire. Based on her strong interest in transitioning and in-depth experience in the area, Anna provided an overview of the multifaceted transition program currently in place at BCCH. She shared helpful information on what parents may expect during the process and how they can help their youth prepare for transition over the years preceding the actual transfer to adult care.
Following Anna’s very informative presentation, Kristine Kerr shared her recent, personal experiences in successfully supporting her son, now 21, through transition to adult care.
